The GLP-1 drugs you hear so much about may have a stronger successor on the way. An experimental weekly shot called UBT251 lowered blood sugar more than semaglutide, the medicine in Ozempic, in adults with type 2 diabetes. It also led to more weight loss. You can’t get it yet, but it points to where diabetes care is headed.

The first GLP-1 drugs copied one gut hormone. Tirzepatide added a second. UBT251 acts on three at once: GLP-1, GIP and glucagon.

How the study worked

This was a phase 2 trial with 211 adults in China who have type 2 diabetes. They were assigned by chance to a weekly shot of UBT251 at one of four dose schedules, to 1 mg of semaglutide, or to a placebo, for 24 weeks. It was published on Oct. 26, 2026, in Cell Metabolism. It was a first look at a fairly short treatment, and larger trials come next.

What they found

The main measure was HbA1c, which reflects your average blood sugar over the past few months. After 24 weeks, UBT251 lowered it by about 2 points, depending on the dose. Semaglutide lowered it by about 1.8 points. The placebo lowered it by less than 1 point.

Here are the exact numbers:

At 24 weeks Change in HbA1c
UBT251, depending on dose -1.98% to -2.16%
Semaglutide 1 mg -1.77%
Placebo -0.66%

UBT251 also lowered fasting blood sugar, blood sugar two hours after meals and waist size, compared with placebo. Weight loss was greater with UBT251 than with either placebo or semaglutide. The most common side effects were stomach problems.

What is a triple agonist?

GLP-1 and GIP are gut hormones that help your body release insulin after meals and help you feel full. Glucagon usually raises blood sugar. In these drugs it is used in a balanced way to help the body use more energy and burn fat, especially in the liver. A drug that activates all three aims to improve blood sugar and weight together.

What you can do today

If you have type 2 diabetes, find your most recent HbA1c result and write it down with the date. At your next visit, ask your doctor what target is right for you. The approved GLP-1 drugs available now are worth asking about too.

Talk about it at home

After you eat, your gut sends little chemical messages that tell your body how to handle sugar. Some new medicines copy one of those messages. This one copies three at once.

Question for the table

Why might three messages work better than one? How could you test that idea fairly?

Questions and answers

What is UBT251?
It's an experimental weekly injection that activates three hormone receptors: GLP-1, GIP and glucagon.
Did it work better than semaglutide?
Yes. HbA1c dropped 1.98% to 2.16% with UBT251 and 1.77% with semaglutide, and weight loss was greater with UBT251.
What are the side effects?
Stomach problems were the most common, as with other drugs in this family.
Can I get it?
Not yet. It's only available in clinical trials, and phase 3 studies are the next step.
