Frailty can improve: a hospital program helped older heart patients

If an older person you love has heart disease and seems weaker or slower lately, here is good news: frailty can get better. In a year-long trial, frail adults aged 70 and older who got a program started in the hospital were less frail and moved better than those who got usual care.

One part of that program was a review of all the medicines a person takes. That means one of the most useful things you can do at home is also one of the simplest.

How the study worked

Researchers in Beijing ran a randomized trial at several hospitals. It included 333 frail inpatients aged 70 or older with cardiovascular disease. By chance, 166 people got the program and 167 got usual care. The people measuring results did not know who was in which group, and everyone was followed for 12 months. The results were published on August 31, 2026 in Nature Communications. The trial took place in one city and some people were lost to follow-up, so the next step is repeating it in more places.

What they found

The researchers tracked two things at 12 months. The first was a frailty score, where lower is better. The program lowered it by about 0.8 points more than usual care did. The exact figure was 0.79 points.

The second was a physical performance test that checks balance, walking speed and how well a person can stand up from a chair. Higher is better. The program group scored about 2 points higher than the usual care group. The exact figure was 2.02 points.

No serious side effects related to the program were seen during the study.

Frailty used to be seen as something that only gets worse with age. This trial shows that it can improve, even in older adults with heart disease. A hospital stay can be a good time to start, because the care team is already close by.

What was in the program?

The program is called RENAP, after its five parts:

  • Rehabilitation exercises
  • Patient education
  • Nutritional guidance
  • Acupoint massage
  • Polypharmacy management, which means a review of the many medicines older patients often take

Frailty means less strength and less reserve in the body. It makes falls, hospital stays and loss of independence more likely. It is common in people with heart disease. The program worked on several fronts at once, and the medicine review was one of them.

What you can do today

Make the list. Write down every medicine, vitamin and supplement your mom or other older relative takes, with the dose and how often. Check the pill bottles, the kitchen cabinet and the bedside table so nothing gets missed. Your pharmacy can help you confirm the prescriptions.

Put the list in your bag and bring it to the next doctor’s visit. Then ask the doctor to look over the whole list together, and to talk about exercise and food that fit her health.

Talk about it at home

Frailty means the body has less strength saved up, a bit like a phone battery that runs down faster. In this study, older people got gentle exercise, good food advice and a check of all their medicines. A year later, they were stronger and steadier than people who did not get the program.

Question for the table

Here is a question to think about like a scientist: why do you think the researchers also followed a second group who got usual care? What would they not have been able to tell without it?
