Intact whole grains lower blood sugar more than whole-grain flour

Here is an easy win for your grocery cart. Next time you buy rice or oats, pick the version that still looks like a grain. In a new trial, adults with type 2 diabetes who ate intact whole grains ended up with lower blood sugar than people who ate the same grains ground into flour.
Both kinds of food count as “whole grain” on the label. But the way the grain is prepared made a real difference. Brown rice, rolled oats and wheat berries came out ahead of whole-grain flour products.
How the study worked
Researchers enrolled 160 adults with type 2 diabetes and assigned each person by chance to one of two groups for 12 weeks. One group got less processed whole grain foods made from wheat, rice and oats. The other got more processed, milled versions of those same three grains. The foods were free and delivered to people’s homes, and participants swapped them in for the grain foods they normally ate. The trial was published Oct. 26, 2026, in the American Journal of Clinical Nutrition, and it compared two kinds of whole grain with each other for 12 weeks.
What they found
Blood sugar, measured by a test called HbA1c, was lower in the intact-grain group. The average difference was 2.2 mmol/mol. Of the 160 people, 143 (89%) gave a blood sample at the end.
Two details make this result easy to trust:
- Weight did not explain it. Body weight did not change differently between the groups.
- The diets matched. Both groups ate similar amounts of whole grains, refined grains and nutrients. The grain foods supplied about 20% of daily energy in each group. The main difference was how processed the grain was.
One more bonus: people eating the less processed grains reported better bowel habits. That was the only other improvement the researchers saw.
What is HbA1c?
HbA1c is a blood test that shows your average blood sugar over roughly the past three months. Sugar in the blood sticks to hemoglobin, the protein that carries oxygen inside red blood cells. Those cells live for a few months, so the test works like a report card for the whole term instead of a single exam. Doctors watch it closely in type 2 diabetes, and lower is better. In the US it is usually reported as a percentage. This trial used millimoles per mole (mmol/mol).
Isn’t whole-grain flour still whole grain?
Yes. Whole-grain flour keeps the bran, the germ and the starchy center. What changes is the structure. Milling grinds the grain into fine particles, so your digestive system can reach the starch faster and turn it into sugar sooner.
Think of a whole apple and applesauce. Same fruit, different experience for your body. An intact grain is closer to the whole apple. The researchers say this points to clearer food labels, since “whole grain” on a package can mean either a kernel or a fine flour.
What you can do today
If you have diabetes, keep your medicines and care plan as they are, and mention any bigger diet changes to your doctor or dietitian. Then make the swap that is easy and tasty. At the store or on your next pantry check, reach for brown rice, rolled or steel-cut oats, barley or wheat berries in place of whole-wheat flour products and instant oat flakes. Put the intact grain in your cart first.

Question for the table
Some foods are made from crushed grains and some from grains that stay whole, a bit like applesauce and a whole apple. Whole things take longer for the body to break down, so sugar reaches the blood more slowly. How could our family find out which keeps us full longer, a whole apple or a glass of apple juice?

Questions and answers
- Are intact whole grains better than whole-wheat flour for blood sugar?
- In this trial of adults with type 2 diabetes, yes. After 12 weeks, HbA1c was 2.2 mmol/mol lower with intact grains than with milled whole grains.
- Did people lose weight eating intact grains?
- No. Body weight did not change differently between the groups, so the lower blood sugar was not explained by weight loss.
- Which foods count as intact whole grains?
- Grains that keep their shape, such as brown rice, rolled or steel-cut oats, barley and wheat berries. Whole-grain flour and bread are milled.
- What is HbA1c?
- A blood test that shows your average blood sugar over roughly the past three months. Doctors use it to track type 2 diabetes, and lower is better.
